When you have experienced a traumatic brain injury or debilitating pain, it can be hard to sustain the day-to-day life activities you’re used to.
We believe that behind every injury is an individual who deserves to live a fulfilling life and make a meaningful contribution to society. Our method was developed to help you transition into your new life and develop the skills and habits that will keep you healthy and happy. We welcome intermittent involvement (as needed) of caregivers and family in our treatment sessions.
Clinical Team: We are a team of providers that includes our Medical Director, Neuropsychologist, Physical Therapists, Occupational Therapists, Speech and Language Pathologists, and affiliated providers as needed
Approach: Our program is designed to use our whole team in providing a day treatment model of comprehensive care. We provide half- or full-day services pending clinical need, typically for a length of time between 4 and 12 weeks.
Basic structure: Each client is assigned a primary team of providers who collaborate with the client in setting goals and track client progress in achieving those positive outcomes.
Services Locations and Areas: Our clinic and Day Program are in Portland. Our Home and Community Rehab program serves the greater Portland area, as well as Eugene and Southwest Washington.
- Counties served in Oregon: Benton, Clackamas, Columbia, Lane, Linn, Marion, Multnomah, Polk, and Yamhill.
- Counties served in Washington: Clark and Cowlitz.
Day Program Outings
The day program at the Portland clinic includes therapy in our community, helping to reintegrate our clients into local community settings, and going out as a group with a team of therapists.
