High-Quality Care and Homelike Comfort
Following a neurological injury or diagnosis, some individuals may require more support than can safely be provided at home. Rehab Without Walls offers two residential care options that combine specialized neurological care with the comfort of a homelike environment, helping individuals achieve the highest possible level of independence.
- Transitional Care provides intensive neuro rehabilitation with 24-hour supervision for individuals working toward returning home or another less restrictive setting.
- Supported Living provides long-term assistance with activities of daily living for individuals who require ongoing support.
Whether someone is staying with us for a short period or long-term, our residential programs are designed to promote comfort, safety, dignity, and independence. We believe receiving expert neurological care shouldn’t mean sacrificing the feeling of home. That’s why we create welcoming environments where individuals and their support networks feel comfortable, supported, and cared for every step of the way.
Transitional Care Program
Our transitional care program bridges the gap between hospitalization and home. Each individual receives a personalized rehabilitation plan based on their unique goals, abilities, and interests, delivered by an interdisciplinary team of neurological rehabilitation specialists.
Therapy may begin in our fully equipped treatment spaces, but as individuals progress, rehabilitation extends into real-life environments where everyday skills can be practiced and regained. From grocery shopping and community outings to navigating public spaces, therapy focuses on preparing individuals for greater independence. Outside of therapy, specially trained staff provide 24-hour support, with one-to-one supervision available when needed for safety.
Is Transitional Care the Right Fit?
Transitional care is best suited for individuals who:
- Need intensive rehabilitation (typically 5-8 hours of therapy each day)
- Require supervision because of behavioral, cognitive, or safety concerns
- Live in areas without access to specialized neuro rehabilitation services
- Need more care than family members can safely provide
- Do not have caregivers available to support recovery at home
- Would require significant home modifications before returning home safely
Some commercial insurance plans and workers’ compensation programs cover transitional care services.

Supported Living Program
Our supported living program is designed for individuals who require ongoing assistance with daily life and are unlikely to live independently in the future. Rather than focusing on intensive rehabilitation, this program emphasizes health, safety, independence, and quality of life in a supportive residential setting.
Residents receive personalized assistance while being encouraged to remain as independent and engaged as possible in their daily routines and community.
Is Supported Living the Right Fit?
Supported living is best suited for individuals who:
- Need ongoing assistance with daily activities because of cognitive and/or physical disabilities
- Require a level of support that family members or caregivers cannot consistently provide
Supported living is available at all Rehab Without Walls residential locations. Unlike our transitional care program, supported living does not typically include intensive rehabilitation services such as physical or speech therapy. Instead, it focuses on providing personalized support that promotes safety, independence, and the highest possible quality of life.
Supported Living Features
Our supported living programs offer specialized services and support, including:
- Assistance with personal care, nutrition, and medication management
- Support with daily living and life skills, including managing finances
- Opportunities to participate in educational, vocational, recreational, and community activities
Some workers’ compensation plans cover supported living services. Select residential locations also accept Medicaid and other state funding.
