Ann Arbor Semi-Independent Living Apartment Program
Our program offers the unique opportunity to experience semi-independent living with the safety net of our dedicated staff to help support participants. Assessing a participant for apartment living can be challenging. At Rehab Without Walls® Neuro Rehabilitation (RWW), we’ve taken a creative approach to adjusting the participant to living semi-independently.
Step One – Staffed Apartment
RWW Ann Arbor has a two-bedroom apartment we use as a staffing apartment. The office is situated in one bedroom, while the master suite is designed for the participant to move in temporarily, while we access the appropriate level of care. As the participant adjusts to living semi independently, they have the safety of the staff present during their stay.
Guidelines are set with the participant on daily living, coming and going, appointments, guests, and general oversight. Participants are encouraged to live their days as they are living on their own. Furnishings are set up for the participant, but we encourage the participant to bring personal items to help make the apartment feel like home.
Step Two – Level of care customized for each participant
As the participant adjusts, a level of care is determined moving forward. RWW will lease a new apartment for the participant and help with moving into and adjusting to the apartment. The goal is to let the person live on their own without feeling alone. As with all our programs, PT/OT/SP, counseling, music therapy, recreational therapy, are all available to the participant, as needed. We will also help with transportation (subject to availability) to and from work, doctor appointments, family visits, and any other appointments as needed.
We offer a unique home and community integration designed for participants to live life to the highest potential, in the least restrictive environment, thus allowing for the greatest results. Our comprehensive neuro program has achieved outcomes well above the industry average, because we treat where the participant lives, works, and plays.
Basic Guidelines for Transitioning Into Semi-Independent Apartment Program
Please note that specific dates will not be given for a move-in date into the apartment program. The date is determined by the Participant’s Treatment Team and the progress of achieving independent goals that were set, in addition to these basic guidelines.
Participants must also be at a minimum Level 6 of Supervision.
Medications
- Know the name, dose, function, route, and administration times of each medication.
- Take all medications with staff visually confirming compliance.
Personal & Environmental Maintenance
- Maintain good personal hygiene without prompts from staff.
- Complete laundry as needed without prompts from staff.
- Create daily meal plan, shopping list, shop adhering to the plan, list and budget, and prepare the meals as planned.
Recreational Activities
- Plan and participate in community social activities each week unsupervised with appropriate interaction. (i.e. movies, dining out, concerts, etc.)
- Pre-plan any personal activities (banking, grocery store, etc.)
- Physical activity at least three times a week
Financial Responsibilities
- Create and maintain a weekly budget.
- Obtain and record receipts for all purchases.
Employment & Volunteering
- Maintain 15 to 20 hours each week of work or volunteer activity on a consistent basis.
- Maintain attendance and promptness according to facility policy.
Behavior Management
- No incidents of verbal aggression. This is defined as yelling, swearing, or threatening other individuals.
- No incidents of property destruction. This is defined as slamming doors, knocking over property, or causing physical damage to the home.
- No incidents of physical aggression. This is defined as hitting, kicking, biting, or any physical contact with another individual causing injury or attempts at these actions.
- No incidents resulting in intervention by public staff (i.e. store or restaurant or police involvement).
- Communicate and interact in an appropriate and respectful manner with all staff, residents, professionals, and the general public verbally and physically.
- Demonstrate ability to channel aggression/anger in an appropriate manner (without slamming doors or objects, destruction of property, etc.).
- Comply with the Program rules/guidelines.
