Comprehensive Vocational Rehabilitation (CVR) Program

SWAN Rehab provides a supportive, rigorous and fun environment to promote self-reliance and independence with the ultimate goal to secure and maintain gainful employment. This is accomplished with a multidisciplinary rehab team that consists of neuropsychology, physical therapy, occupational therapy, speech therapy, social services and a vocational specialist.

Each patient is evaluated and works closely with their team to develop a program that is custom to their needs and delivered with one-on-one and group therapy. SWAN Rehab uses the latest technology and clinical techniques to support the patient in achieving their goals. Our team of therapists provides services in the community, as well as the clinic, by working with clients in their home and at the job site in order to integrate everyday life and challenges into therapy.

SWAN Rehab specializes in the complex diagnoses of traumatic brain injury, spinal cord injury, stroke and other neurological disorders.

Vocational Services*
Vocational Services focuses on skills to obtain and maintain work. Therapists assist with learning professional behaviors, career assessments, resume writing, interview skills and job search. We offer onsite supervision and collaboration with the employer to provide education about accommodations and about the injury.

Neuropsychology*
Neuropsychology team will evaluate neurocognitive and psychological strengths and limitations. We will provide cognitive retraining and focus to improve cognition. Neuropsychology also focuses on adjustment to disability as it relates to personal and professional life.

Physical Therapy
Physical Therapy helps improve your ability to move safely around your home and workplace. They work on improving functions like strength, flexibility, range of motion, endurance, balance, coordination and safety awareness. They also help you to compensate for permanent physical and cognitive disabilities by training you to use adaptive equipment and other strategies to safely navigate home, community and work environments

Speech Therapy
Speech is designed to improve communication in both personal life and workplace. Speech therapy also works to improve memory, speed of processing and other cognitive deficits. Therapy is individualized to the person but can include making phone calls, having conversations, or other daily activities to improve communication skills.

Occupational Therapy
Occupational Therapists help with skills and strategies needed to improve organization, managing home life to return to work. They also provide assistance with compensations to improve cognition, memory, organization, and attention. Technology can be an important part of independence for return to work.

Social Work
Social work focuses on supportive counseling addressing adjustment to disability. In addition, social workers assist individuals and families to obtain community resources as well as understand their benefits (SSI, SSDI, AHCCCS, etc.) for return to work.
